What inspired you to take on this challenge?

It is really helpful for us to see the wider context of a supporter’s decision to fundraise for us, and allows us to communicate with each supporter in a more personal and sensitive way. It is, however, not compulsory to share your story.

Our running tops cost £12 to produce. If you would like to make a donation towards the cost of your running top so that even more of your fundraising can benefit Bristol Children’s Hospital, you can do this via the donate button after submitting this form.

Consent

I agree that, to the best of my knowledge, I am in a fit state of health for the purposes of this event, that I will be undertaking the necessary training and preparation, seeking medical advice where appropriate and understand that I am taking part entirely at my own risk.

I confirm that adhere to the skydive guidelines and that I am at least 16 years old (and that if I am under 18 years old, I can provide parental consent). I also understand that if I am over 15 stone (95kg) fulled clothed, an extra charge of £20 per stone will be added.

I understand that by registering for this event, I am giving my consent for any photographs taken at the event in which I feature to be used by The Grand Appeal in any future media reporting or promotion of the event across social media, print and our website, and that it is my responsibility to inform the charity should I not wish to be featured.
